Support Coordinator

The Support Coordinator provides quality supports and services through the development, training and ongoing monitoring of the lifestyle planning process. They work in unison with the Supported Individual, CORE Team, Support Staff, and family to assess needs and create individualized plans of care. The Support Coordinator will work in unison with the Residential Manager to ensure proper care plan implementation, tracking and employee/client interaction.
